Rapamycin is longevity's best cautionary tale — because its evidence is genuinely good

Rapamycin deserves the attention it gets: it is the most credible pharmaceutical candidate in ageing biology. It also delivers, within a single research programme, the clearest demonstration of why early human results should not be extrapolated — which is precisely what much of the longevity community does with it.
The rise: two encouraging trials
In 2014 and 2018, randomised trials of low-dose mTOR inhibitors in older adults reported improved responses to influenza vaccination and fewer self-reported infections. Real endpoints, real randomisation, real promise — these results are a large part of why rapamycin became the centre of the longevity-medicine world.
The fall: the phase 3 that followed
The same scientific programme then ran the definitive test: a phase 3 trial of an mTOR inhibitor against clinically confirmed respiratory illness in older adults. It failed. Not ambiguously — the primary endpoint was missed, and the development programme for that indication ended.
This sequence — phase 2 success, phase 3 failure — is one of the most common patterns in drug development. Most people taking rapamycin off-label for longevity are, in evidence terms, acting on the phase 2 data while ignoring the phase 3.
Where that leaves the longevity use
Our rubric grades rapamycin's immune-function claim as mixed: credible randomised evidence on both sides. Its human lifespan claim has no direct trial evidence at all, and the first year of the PEARL safety study, while reassuring on tolerability, was not designed to answer it. Rapamycin remains the intervention most worth watching in this field — watching being the operative word.
